You or your healthcare provider can submit an application for medical healthcare from Radboudumc.
- Please complete our application form

Once we receive your application, we first send it to the relevant medical department for assessment.
Once your application has been assessed, we will let you know whether you can come in for treatment, or whether we are unable to meet your healthcare needs. -
Depending on your application, we may require additional information from you, such as personal and/or medical data. We will let you know whether that is the case.

Once your application has been approved, our Finances Department (Costs of Care) will contact you concerning the financial documents required in your situation.

Prior to your appointment and/or treatment, we will ask you to prove in advance that the healthcare expenses to be incurred can be paid. This can be done through your health insurance or by means of a letter of guarantee or advance payment.

Once your application for healthcare from Radboudumc has been approved and if the necessary financial requirements are met, an appointment will be scheduled. You will receive an appointment confirmation by email and/or post.

What should you bring along?
- Your identification (ID): a valid passport or a valid ID card from an EU/EEA country.
- An overview of the medicines you are currently taking.
- The confirmation of your appointment (appointment letter). You can use the bar code on this letter to register at the registration columns at the main entrance.

During your first appointment (online or in person) at Radboudumc, you are required to identify yourself. Identification is mandatory for anyone receiving medical healthcare in the Netherlands, including children.
You can identify yourself with any legally valid form of identification: a valid passport or a valid ID card from an EU/EEA country. You will receive further notice about this.

Following your treatment, examination or admission, we will ensure that you receive proper aftercare. We usually do not provide this aftercare ourselves, but rather delegate it to your physician in your home country or to other healthcare providers involved.
The right way to organize aftercare is a medical decision that differs from case to case.
